Objetivo: Descrever a vivência de acadêmicas de enfermagem durante o estágio
supervisionado. Métodos: Relato de experiência do tipo descritivo de observações
de pacientes com cateteres venosos centrais em dois centros de terapia intensiva de
um hospital estadual na cidade de Serra, estado do Espírito Santo. Resultados e
discussão: A média de idade foi de 49 anos, 70% do sexo masculino e 100%
apresentavam risco para infecção primária da corrente sanguínea de acordo com as
observações. Há Bundle tanto na evolução do enfermeiro quanto a beira leito. A
existência de condição clínica instável necessita de cuidados intensivos devido
algumas comorbidades como hipertensão e insuficiência renal. Usa-se clorexidina
alcóolica no curativo da punção, luva estéril, Transpore e gazes diariamente após o
banho. Práticas imediatistas sem avaliação prévia foram observadas através da
infusão de novos fármacos. Conclusões: Houve interpretação das diferentes
dinâmicas do processo de trabalho dos intensivistas. A avaliação do sítio de punção
durante as trocas de curativos foi possível. A instituição de saúde deve se atentar
acerca da presença de infecções da corrente sanguínea pelo dispositivo. Os itens do
Bundle na prevenção de infecção pelo cateter venoso são primordiais. O critério
para a escolha do antibiótico requer uma maior criteriosidade. Há relação indireta
entre comorbidades e cateter venoso central, onde impulsiona o risco de infecção
devido excesso de sua manipulação pela equipe. Sugerimos investimento em
recursos humanos qualificado através de treinamentos assistenciais e que este
estudo sirva de contribuição para novas pesquisas que abarcam tais vivências. |

Objective: To describe the experience of nursing students during the supervised
internship. Methods: descriptive experience report based on observations of patients
with central venous catheters in two intensive care centers in a state hospital in the
city of Serra in the state of Espírito Santo. Results and discussion: The average
age was 49 years old, 70% male, and 100% were at risk for primary bloodstream
infection according to the observations. There is a Bundle both in the evolution of the
nurse and at the bedside. The existence of an unstable clinical condition requires
intensive care due to some comorbidities such as hypertension and renal failure.
Alcohol chlorhexidine is used in the dressing of the puncture, sterile glove, Transport,
and gauze daily after the bath. Immediate practices without previous evaluation were
observed through the infusion of new drugs. Conclusions: there was an
interpretation of the different dynamics of the intensivists' work process. The
assessment of the puncture site during dressing changes was possible. The
healthcare institution should pay attention to the presence of bloodstream infections
by the device. Bundle items in preventing venous catheter infection are paramount.
The criterion for choosing the antibiotic requires greater criteria. There is an indirect
relationship between comorbidities and central venous catheter, where it encourages
the risk of infection due to excessive manipulation of the catheter by the team. We
suggest investment and qualified human resources through assistance training and
that this study serves as a contribution to new research that encompasses such
experiences. |
